Ground Transportation Permit Applications

Ground Transportation Permit application packages should be emailed to oakGTpermits@portoakland.com

Instructions For Email Correspondence

Ground Transportation Permit application package email correspondence must include the following information:

  • Your company name in the subject line.
  • Include the following in the body of the email:
  • Full Name
  • Full Address
  • Contact Phone Number
  • Your Email address

Ground Transportation Permit Application (typed) with applicable Attachments (reference GENERAL REQUIREMENTS page for all listed items). 

General Application

  1. Sub-carrier Application (if applicable) with Notarized Copy of Contract Between Subcarrier and Main Carrier
  2. Driver Roster (with photocopies of valid CA driver license)
  3. Equipment Statement (with photocopies vehicle registration)
  4. Insurance Certificate (with Workers Compensation if applicable)
  5. Any other items listed to submit a completed application package

Please note, sending more than one email may delay our response.  Do not email applications to GT staff.
