New Badge Applicant Checklist

Step 1: Application and Fingerprinting

  1. Authorized Signer submits badge application. 
    Ensure accuracy and appropriate levels of clearance and/or endorsements are assigned. An email from the “OAK ID Badging Office” will be sent to Authorized Signer and applicant after submittal.

    Authorized Signer will verify that applicant has two unexpired acceptable government issued identifications which MUST be provided at all appointments. Both IDs should have identical spelling and placement of applicant’s name. Please review the OAK I-9 form guidelines.

    * If Customs Clearance is needed, applicant must provide the CBP packet at time of Fingerprint Appointment. 

  2. Schedule Fingerprinting appointment. 
    ID Badging Office Appointment Scheduler (also provided in your email). 
    Scheduled appointment must be made within 30 days of application submittal.

  3. Applicant completes and signs forms.
    The email from the “OAK ID Badging Office” contains links to forms required to be completed and signed before their appointment.

  4. Arrive on time to your appointment and provide your two (2) pieces of identification. 
    Those arriving more than 15 minutes late or without proper ID’s may be asked to reschedule the appointment. 

  5. Payment is due at time of appointment: 
    Fingerprint Fee: $50
    Badge Fee (may be paid at the next appointment): $25

Step 2: Computer Based Training and Badge Issuance

  1. Authorized Signer receives the clearance notification.
    Allow 14 to 21 days for the ID Badging Office to notify the Authorized Signer whether the applicant has passed the Criminal History Records Check (CHRC) and/or Security Threat Assessment (STA).Scheduled appointment and training must be completed within 30 business days.
  2. Authorized Signer informs applicant of clearance notification and makes an appointment to complete training.

  3. Arrive on time to your appointment and provide your two (2) pieces of identification.
    If you are more than 15 minutes late, you may need to reschedule the appointment.
  4. Payment due at time of appointment:
    Badge Fee (if not already paid): $25
    Badge issued after completion.

  • Invoice (If your company has an invoice account with OAK)
